What forms do I need?

Most major medical plans do cover chiropractic care. We are happy to check your benefits for you. Currently, we are in-network with the following major insurance companies:

At this time, we are NOT considered in-network with Kaiser, Cigna, Medicaid or Anthem HealthKeepers. If you don't see your plan listed, please contact us to ask, as there are many smaller plans that we may not have listed. 

 

Please note that some plans require you to attain a referral from your Primary Care Physician. We can aid in this process.

 

For those that do not have health  insurance, we do offer cash plans. We are happy to answer any questions you may have regarding insurance and benefits.
